Self-Attestation Forms

All students, staff and faculty members are required to fill this out.


  • Announced on Aug. 18, Gov. Jay Inslee announced several key changes that affect TCC employees. The changes are:

    • All employees must be fully vaccinated by October 18, 2021.
      • Fully vaccinated means, at least 2 weeks have passed since receiving the second dose of a 2-dose vaccine series, or at least 2 weeks after receiving a single-dose vaccine.
      • This requirement is a condition of continued employment.
      • This applies to all employees, including full-time, part-time, and student employees, as well as those who are teaching or working remotely.
      • Proof of vaccination will be required. (Additional information to come.)
      • Medical and religious exemptions will be offered; needed documentation must be provided. (Additional information to come)
      • No personal/philosophical exemptions will be allowed.
      • A self-attestation form must be completed by every employee by October 18. Employees are strongly encouraged to complete the form as soon as possible after receiving their final vaccine dose.
        • Please note: The self-attestation form for employees will be updated within a week. Directions will be updated as well. 

For Students

  • Students  who attends classes both on-campus and online will need to attest to full vaccination status or declare a vaccination exemption. Attestation forms must be completed by Sept. 17.    
  • The self-attestation process will be quick and easy. Fully vaccinated students can fill out the attestation form online. You’ll need to include your vaccination dates, but at this time we don’t need a copy of student vaccination cards.
  • Students who require a medical, religious or personal/philosophical vaccination exemption will need to fill out the self-attestation form and declare an exemption. 

Attest to your vax status

The form will require you to follow college safety protocols and acknowledge your understanding that the information you have provided is accurate and true, and the information provided may be subject to further verification and enforcement.

Please note the following important information: 

Limited disclosure regarding vaccination status is allowed, as long as the disclosure only serves the need to ensure that vaccination policies and face covering requirements are being followed. Vaccine information will be accessible to specific individuals in Human Resources (employees) and Student Affairs (students).  Note that under FERPA, institutions must keep a log of persons who receive access to student vaccine records per 34 CFR 99.32.
